CHICAGO, US: Additional oral hygiene measures are usually recommended to supplement daily toothbrushing in orthodontic patients because they are prone to plaque accumulation and caries developing around fixed appliances. At the Chicago Dental Society Midwinter Meeting, Swiss oral healthcare products specialist vVARDIS is currently showcasing Curodont Repair Fluoride Plus and Curodont Protect that help combat these issues.
According to several studies, about 50% of patients undergoing orthodontic treatment show initial carious lesions. Commonly adopted management strategies to motivate patients to maintain better oral hygiene include using interdental brushes and mouthwashes and applying fluoride varnish and remineralizing formulations. However, obtaining patients’ compliance with new or additional oral hygiene practices can be challenging. Therefore, precise and targeted treatment of early carious lesions in-office to maintain the tooth’s integrity and supplementation with a home use formulation could offer a solution.
Curodont Repair Fluoride Plus is a noninvasive treatment for initial carious lesions through guided enamel remineralization. It is a peptide-based patented formula derived from natural amino acids that self-assembles when introduced into an acidic medium, forming a matrix that acts as a scaffold to guide tooth remineralization by attracting calcium and phosphate from the saliva. It is a low-viscosity transparent liquid that can be applied to initial carious lesions in an 8- to 10-minute in-office procedure by dentists or hygienists. A recently published article reported a treatment success rate of 96.4% sustained over three years after the initial use of Curodont Repair Fluoride Plus to treat early caries.
To support the in-office treatment of these lesions with Curodont Repair Fluoride Plus, Curodont Protect is recommended for home use. It is an anti-cariogenic transparent dental gel that contains stabilized stannous fluoride as the active ingredient and employs vVARDIS’s proprietary technology. Curodont Protect stabilizes the active ingredient on the tooth surface and supports the uptake of calcium and phosphate ions from the saliva, increasing their availability for enamel remineralization. It is not only an ideal complement to Curodont Repair Fluoride Plus for the treatment of initial lesions, but can be prescribed at the start of orthodontic treatment for use around brackets to prevent caries developing.
An ideal protocol is to provide Curodont Protect for home use twice a week when the brackets are bonded on the teeth. During patient recall appointments every four to six weeks for adjustments of appliances, the clinician can do a thorough assessment to check for any developing lesions and treat them concurrently with Curodont Repair Fluoride Plus combined with home use of Curodont Protect once a day for one week on the treated lesions, continuing with twice-weekly application to unaffected teeth. This way, clinicians can maintain an active surveillance for prevention of and early intervention for caries, avoiding the risk of lesions deepening, widening and becoming more challenging to treat.
Clear aligners mitigate some of the difficulties of oral hygiene maintenance associated with fixed appliances. However, a study has shown that aligners that are worn for 20–22 hours a day act as a barrier between the tooth surface and saliva, limiting the flow of saliva that is so critical for maintaining the remineralization–demineralization equilibrium in the oral cavity.
Together, Curodont Repair Fluoride Plus and Curodont Protect form a beneficial system for the prevention and treatment of initial caries due to improper oral hygiene maintenance during treatment with fixed orthodontic appliances and aligners.
